Record Transfers
Please email vdentalcenter@gmail.com to request to have your dental record(s) transferred to your new dentist. Please provide us with the patient’s name, birthday, the relationship of the person requesting it (minor patients only), and the new dental office’s email address. Please note it may take up to 2 weeks to process any of these requests.
